    Trench’s Health Insurance Tips: Diagnosis Coding

    Today’s healthcare pet peeve du jour is diagnoses and the coding of your medical claim.

    A lot of our patients come in just for a routine check-up, a cancer screening, or they have a family history of some disease. In all 3 of those cases, an office visit or procedure is considered “routine” because an actual disease has yet to be treated, and no other complaints have been presented. In some of those cases, health insurance companies do not cover routine services. So, let’s take a look at a little hypothetical situation, shall we?

    The patient comes to see the doctor for a cancer screening.

    The doctor uses the diagnosis code on the claim for cancer screening.

    Insurance company denies the claim because they do not cover routine procedures.

    The patient gets a statement from the insurance company saying they are not paying for the visit.

    Patient calls the insurance company to ask them why they didn’t pay.

    This part is important, so pay attention…The insurance company tells the patient that they do not cover routine procedures, but if your doctor had coded it as non-routine, then it would have been covered.

    What the patient hears is, “Your doctor coded it wrong.”

    The patient then calls me and wastes my time telling me that the doctor coded the claim wrong, and it needs to be recoded.

    Here’s where I bust the myth.

    The doctor did not code it wrong. In 99% of all cases, the doctor coded it correctly with the information provided from the patient.

    With the information that we have received from the patient, we can not recode the diagnosis as anything else in order to get the claim paid. That is known as INSURANCE FRAUD. I am not willing to go to jail just so you can get out of paying your bill. Health insurance is not an absolute. No matter how good your coverage is, you will eventually have to pay out-of-pocket. It’s inevitable.

    Do me and yourself a favor. Every year, you get a booklet from your employer that goes into great detail about your health insurance coverage. READ IT! LEARN IT! LIVE IT! And stop bothering me with your assclownic questions.

    Eric Harris’ Parents Selling Home Near Columbine:

    The parents of Eric Harris have put their house up for sale. The house where guns and bombs were stored. The house where all it would have taken was for Wayne and Kathy Harris to look in on their son and Columbine would have been prevented.

    They’re asking roughly $270,000 for it. The realtor does not have to disclose what happened in the house. Two realtors are not allowed to show the property. That would be Rich Petrone, the stepfather of Danny Rohrbough who was killed at Columbine, and Randy Brown, the father of Brooks Brown who was threatened by Eric Harris.

    I wonder why they wouldn’t want those two showing the house. Maybe because this way they don’t have to look them in the eye and explain why they failed to prevent their son from killing one of theirs.

    If I had FU money, I’d buy the house and have it immediately demolished. Hopefully, someone who does have that kind of money will do the same.

    There was a chemical spill inside Columbine High yesterday. It was an accident that injured 3 students and a teacher. While I do not mean to belittle the injuries that happened, this should have been nothing more than a local story in Colorado.

    The Denver Post ran with an AP article that treats it as such. Other news outlets didn’t. The Daily camera ran with the same AP article the Post did, but added this little blurb at the end…

    Columbine was the scene of the nation’s worst school shooting on April 20, 1999, when Eric Harris and Dylan Klebold shot and killed 12 students and a teacher before committing suicide.

    You don’t say I hadn’t heard that. The Washington Times was even worse. It ran with a UPI article that started with…

    Columbine High School, site of the worst U.S. school shooting, was evacuated after a chemical spill that left a teacher slightly injured.

    And ended with…

    On April 20, 1999, Eric Harris and Dylan Klebold shot and killed 12 students and a teacher before committing suicide.

    This story has nothing to do with the Columbine killings. The only thing they have in common is that they took place at the same high school.

    Is it always necessary to bring up Harris and Klebold when talking about a situation that does not involve them in any way shape or form? I think we all know what happened at Columbine High. Apparently, the media thinks we’re so stupid we need reminding.

    Columbine evidence is ruled public information:

    The sub-headline says it all, doesn’t it. “The families of the two killers wanted the diaries and videos kept private. A slain student’s parents feel differently.” I don’t understand why this stuff wasn’t made public a while ago. The only way I could see them not releasing the evidence into the public record is if there was still a criminal investigation going on against someone involved with the Columbine shootings.

    Personally, I don’t like the idea that the media thinks they have the right to view anything they want, like the Dale Earnhardt autopsy photos, but in this case, they’re right. The families of the 13 victims have every right to view every last piece of this material. But of course, not everyone wants the material released. Care to take a guess who?

    The killers’ parents have fought release of the documents, saying it might trigger copycat episodes. Their attorneys didn’t return calls.

    Translation: Please don’t release these records, or you’ll see what lousy parents we are. “Might be” copycat episodes? Where the hell have they been? Since their two mutant offspring killed 13 people, there have been scores of copycat episodes. Luckily, most were averted before anybody could be hurt or killed. Not to mention the scores of Klebold and Harris worshiping mutants that leave their bouts of mental masturbation on my site.

    It’s obvious that the Harris’ and especially the Klebolds don’t want it to get out that guns were being purchased and bombs were being manufactured under their watch. Do you think I’m the only one who thinks that? Think again…

    Sue Petrone, mother of slain student Daniel Rohrbough, welcomed the ruling.

    “The purpose of the information is just so people can learn,” she said. “If they see something like this in their own home, writings like this, these are things people need to be concerned about.”

    Daniel’s father, Brian Rohrbough, said the materials’ release will shed light on the Harris and Klebold households.

    “I think from what we can see from all these items is what was going on inside the houses. It gives us a glimpse of the Harris and Klebold homes. It is important to see what was going on,” Rohrbough said.

    He said the parents of both killers have spread the myth that they never saw anything that would have tipped them off to their sons’ plans.

    Here is the most important quote as far as I’m concerned…

    He said that when people see the videos, it will give them a clear picture of what the homes were really like. And other parents will know what to look for, Rohrbough said.

    “It is important to learn the truth,” he said.

    That’s the important part right there. So other parents will know what to look for. Unfortunately, as we keep finding out, a lot of parents have no clue, or they don’t want to be too hard on their kids like our parents were, so many of the kids become unsupervised at most times. Or their parents really don’t care. Don’t be one of those parents. Too many parents have the “Not My Kid” syndrome. If you are one of those parents, you better find a cure quick before your kid becomes “one of those kids”. The Harrises and the Klebolds didn’t. Now 13 people are dead because of it.

    I originally told you about Al DeGuzman here. He was the Klebold and Harris worshiping mutant that was sentenced to 80 years for planning to blow up his community college. Turns out he really was a coward like his heroes. He hung himself in his jail cell. I guess 80 years is really only 2 1/2 months. There’s an old saying that says don’t do the crime if you can’t do the time. By the way, thanks for saving the taxpayers some money.

    If you had paid attention, you’d know that my using the pseudonym of TheTrenchcoat pre-dated any negative stigma that might be carried with it. So, if you think about it, I was the original. Anyone who calls themselves a Rebel really isn’t one and is just another conforming sheep.

    And again I ask where is your educational site? The only thing I’ve learned from you so far is how obvious it is that no one cares about you, and that’s just sad. I have no idea what real life is like? Whatever sister. You’re the one living in a blood-soaked fantasy world where killers are heroes.     I feel like I’m repeating myself here, but Harris and Klebold were cold-blooded killers. They killed 13 innocent people. Most of us were harassed in school, but we didn’t kill anybody over it. Eric Harris’ parents failed in their job as parents. They should be forcibly sterilized so they don’t reproduce again. Harris and Klebold are not people to be worshiped or idolized or even sympathized. What makes me even sicker is that there are other people out there like this sick bitch that have signed the guestbook. The only solace I take in a site like this is that Harris and Klebold are burning in hell.

    Trench’s Health Insurance Tips: Deductibles

    Here’s a little health insurance tip from me to you. It will make my life easier and make you look like less of an assclown.

    Say your health insurance policy has a $500 deductible. And say that you have a condition that requires you to see multiple practices (i.e. primary care, specialist, and hospital). This would require you to file 3 claims to your insurance company.

    Now say the hospital asked you to pay your $500 deductible up front and you do. The claims from all 3 practices get filed with the insurance company. However, the insurance company gets the specialist’s office’s claim first and takes out the $500 deductible from that claim, leaving you with a $500 balance at the specialist’s office.

    What’s that, you say? You shouldn’t have a specialist’s bill because you already paid your deductible to the hospital? Well, let me be the first to say, YOU STUPID ASSCLOWN!!!! Run up to the nearest wall and slam your head into it for being so stupid.

    Let it be known now, forever and throughout perpetuity that the insurance companies not only don’t know who you paid your deductible to, they don’t really care either. The insurance company will take out your deductible from whatever claim they receive first.

    Now that I’ve made you smarter, here’s what you can do if you paid one practice up front and the insurance company took out your deductible from a different claim. If you paid up front to one practice and the deductible was applied to another, you should have a credit at the practice you paid up front.

    DO NOT call the insurance company about it. DO NOT call the practice that is billing you for the deductible. Neither of those places can do anything about it. DO call the practice or facility that you have a credit with. They are not going to call you.

    After you pay a practice up front, it becomes your responsibility to get any credit back. Also, the practice that you have a balance with is not going to wait around for you to collect your credit. They will keep billing you and possibly send you to collections unless they receive payment. They are well within their rights to do so.

    Some of you talk about bullying like you understand it. Klebold and Harris killed themselves, that’s what real bullying does to you. A lot of people who get bullied to far kill themselves, the only difference with Dylan Klebold and Eric Harris is that they took some of the fuckers with them. And as for the ‘innocents’, to victims of bullying the crowd that laugh along with the bullies are just as bad as the bully themselves. Trenchcoat you are just another shithead who doesn’t have a clue what he is talking about. Me I’d treat bullies the way Hitler treated the Jews, but a lot crueller, and I‘d probably sell the remains back to the schools as food. And surely its the parents that allow their kids to grow up as bullies that should be forbidden from breeding.

    Klebold and Harris are/were almost hero’s.

    :twisted:Speaking as someone who has been bullied, the life of a bully is worth less than a single piece of my sh*t, after I‘ve flushed it, and the only reason I‘m not hacking them to pieces is that I don’t want to annoy the police or go to prison or anything!     Tell me O wise one what I don’t understand about bullying. I was bullied my entire time in school. But guess what, Sparky? I got over it. Harris and Klebold were/are not almost heroes. They would have been heroes if they had done something rational and legal to combat bullying. Instead, they’re just two pathetic cowards burning in hell who are worshiped by someone who is in obvious need of psychiatric help and whose parents obviously don’t care enough about their child’s life.
